Homebound Delivery Service Application

The Palatine Public Library District offers materials delivery service to patrons who are valid Library District cardholders and are unable to come to the Library for a minimum of 30 days due to a temporary or permanent disabling condition.

Eligible patrons may complete a Homebound Delivery Service application form and submit it to the Homebound Services Coordinator.
